The $100 rebate form came from Cox with lettering on the envelope stating "Last Chance!" I hadn't had any other chances that I can recall. The form states that the order must be made 11/21/06 - 3/2/07 so it's not to late for those who ordered weeks ago. The form with 1st bill must be postmarked by 3/31.
