I would be surprised if it had not.

This same problem is becoming normal at the indian casinos around
Dallas. The paper here has had storys and if I remember correctly
many, like double digit # or jackpots have been denined in the past
few months because of "machine errors". No regulation, no recourse,
etc.. I would never put a dime into an Indian casino slot until they
agree to regulation.

It has also happened numerous times at the Oklahoma Tribal Casinos just north of Dallas. I'll save my money for Vegas...

I was countering the implication that this only happens at Indian
casinos, google tells me that it has occured in LV (on a Megabucks!
ouch!), Reno, a Harrah's run tribal casino, several in Canada, and in
more than one midwestern state. I am going to speculate it has
happened more than I can locate news articles about it. Google "slot
machine malfunction".

The big difference is that in U.S. casinos, there is always an avenue of appeal.
