Does anyone know if the TITO coupon you take out of the machine has been
encoded (in the bar code) with your slot club card number (assuming, of course,
you are using it at the time)

I can't say absolutely but since the TITO and card readers are independent
devices controlled by different pieces of software it is extremely
unlikely. Particularly since the TITO system would need to be able to
access information from any one of several different, incompatible card
reading systems.

Actually, it strikes me that this would be extremely easy to do. As for different pieces of software interacting, that is common enough that any knowledgeable programmer would have no problem.

I don't know whether they are doing this or not. If they really wanted to, with the right hardware and programming they could have the eye in the sky take your picture and print it on the ticket, too.

In theory, the casino could also encode information such as wheter a
video poker player is skilled/advantage, what kind of machine the
player was playing, etc.
