9a. TIPPING?

$20 probably appropriate. If you figure you'll probably tip the bellman $5 or $10 to bring up the bags to your room for four people, and he only spends 10-15 minutes on the task, and then figure the driver needs to drive to the airport, park in the designated area, and await your flight (and even if they check scheduled arrival time, may need to sit around for a while), and add that "down time" to the driver's actual time with you, plus the fact that he/she will probably also be lifting the bags at least once or twice, it seems pretty fair.
Most of the drivers meet you at or just before the baggage claim area and help you get your bags off the carousel, get them to the car for you, etc., so there's more service than just the drive. If by some chance you get an unusually abbreviated service that only consists of the drive, a smaller tip might be appropriate, but if you get the full service, $20 is fair.
Remember, most upscale casinos would charge you $30-50 for their limo if it's not comped, and most limo services are in the $20-40 range (if I remember correctly) before you tip the driver, so you're still way ahead at $20.
