re playing on husband's card

Does this mean that if I leave my card in the machine by mistake, adn someone else hits a jackpot on it, I still get the tax form?

I can't speak from personal experience (yet), but I'm told that if you
win a W2-G reportable jackpot with your spouse's card in, the casino
requires the W2-G be issued in the spouses name (and if they're not
present, they'll need to claim the win at the cage when on site).

In fact, after the first reported jackpot, spitting out the W2-G from
the system is near automatic. To my irritation, photo ID is rarely
requested - they just take my card. Makes me nervous having misplaced
at least one card over the years.

At Sam's Town, this week, my wife hit for almost $11000 with MY card
in the machine. Absoutely NO problem. A player's card is NOT
required to be paid.

I have hit many taxables with my spouse's card in. I might be
playing with my spouses card because she has multiple points that
particular day and I don't. For those that think this is a problem,
my casino host suggested it to me several years ago. I had a good
friend playing on my card last week and he hit a taxable with it in.
He got the W2g in his name, BUT, I am pretty sure that my end of the
year statement next year will have that handpay on my account. I say
this because in 2003 I hit four taxables with my wife's card in,
first time this happened, I noticed that the W2g had my wife's name
with my SS# on the paperwork. I had them re-do it. After that
experience, whenever I hit a taxable with my spouse's card in, I
asked the floor people to make sure that the W2g was issued in my
name with my SS# on it. At the end of the year, the handpays were
still listed on my wife's end of the year statement.
