that had the numbers pulled via an RNG instead of a live drawing. Somebody
figured out what was happeneing and won $200,000 Canadian each of the first
three plays of the game. The casino knew about the problem prior to the
incident but continued to offer the game anyway refused to pay. The player
sued and won. The casino had to pay the entire $600,000.

I'm not sure if this is "urban legend", but , I heard this story:
Several years ago, on one of the river-boats that would turn-off all
the slot machines at the end of the gaming day and then restart the
machines at the same time each day. Well, the prng took the seed
number from ths slot machine's internal clock, hence each day the
machines would run through the same initial cycle, and hence could be
