Dear anon,

Your second question would require multiple answers for me to answer
accurately.

I started at the Gamblers Book store in 1995.
- bought Lenny Frome's book - used it for first trip.
  (hit a royal, was hooked).
- next trip back, bought VPTUTOR.
- finally, taught myself - using VPT to create my own strategies for
20 or 30 games.

So, what contibuted the ??MOST??

Answer - self-taught - BUT - I couldn't have done it without the
catalyst of Frome's book, and the utility of Catlin's software.

It would make sense (in my case) to have the question re-worded from
"What contributed most" to "What contributed heavily", and support
multiple answers.
