The credit meter shows a balance of $1,355. It looks like the machine credited the
player the $1,250. No tax forms for this hit?

This photo was taken before the attendant made the hand pay.
The hand pay will include a W-2G, for sure.
When attendant returns with the cash and the form, you sign the form,
get your copy, get the cash and then the attendant resets the frozen
machine, which will subtract $1250 from the cred total.
Typically, they will ask you to 'play off' the hand and watch you do it.
Nice dealt hand and sweet multiplier.
