They didn't give me any tax forms when I cashed in the $500 vouchers
but of course we keep a log of our wins and losses for tax purposes.

if I am not mistaken, and of course I could be, SAR in a casino is for
cash transactions of <i>at least</i> $5000.00.

And, the circumstances of this win are not that <i>suspicious</i>,
therefore, no SAR.

3 grand is the "official" limit, obviously people are gonna try to
pass 3 grand as just under the legal limit, so there is going to be a
lower actual enforcement limit to try to catch these people
