My wife and I have been playing video poker together for about 15 years.
We travel to either Vegas, Reno/Tahoe, or New Orleans about 6 times each year from our home in a non gambling state (SC), and stay for anywhere from 3 to 14 days each trip.
We've each been dealt a Royal Flush once.
In my wife's case it happened at The Hyatt Grand Lodge in Incline Village, NV on a $1 progressive single line machine playing DW for just over $8,000.
In my case it happened at Harrahs New Orleans, also on a single line $1 progressive machine for just over $6,000.
My dealt Royal was a sequential Royal in Spades playing DDB. Unfortunately I wasn't playing on a machine that paid a premium for a sequential Royal.
Neither of us has ever gotten a Royal on the redraw after discarding all 5 cards.
As an aside, I was playing on a triple play Quick Quads machine at the M in Vegas last week and was dealt four to the Royal. On the redraw I hit two Royals. I know that's not that terribly rare, but it surely was an unexpected and pleasant surprise.

Sorry, i am having a hard time with the Yahoo format. Some context: I wrote that I got a royal on the redraw after discarding all five cards. Some folks were speculating on the odds of this happening, from the point of view that the cards discarded were all low cards. I just wanted to state that I had been playing FPDW, so some of the cards could have been high ones. The only thing we know for sure about the composition of the remainder in the deck is that there were 5 suited royal cards. Also, I would not have discarded deuces.
When I got a dealt sequential royal a few weeks ago, I asked about the chances of this happening. There are people in this group that are capable of the math (not me!). Dunbar also came up with the chances of this at close to 1 in 77 million.
i agree that this would make a great promo for a casino. They could purchase insurance to protect them in the event someone hit.

"Sorry, i am having a hard time with the Yahoo format." Yes, the "new" yahoo format sucks. Wonder if it is a reason the VPFREE traffic is way down. That's bad for AVP players. The "new" format is ~ a year old and is still tedious. The good news for yahoo groups readers is it's not as bad as the "new" yahoo mail which is Awful.

My mom and dad and spouse have all been dealt royals. I have yet to experience that feeling. Hope that it happens one of these days.

My best dealt hand was 4 3's DDB with no kicker on a 100 play 1 cent multiplay. The final result was $605. Of course I was betting $5 per play.
