I have commented on this several times. I think it's EXTREMELY unlikely that anyone has ever received the same card back on the draw on a machine from a manufacturer that is approved to supply gaming machines in Nevada. The possible exception would be for a newly designed machine, such as the Odyssey 4-play machines that did have a problem with duplicate cards when they were first introduced, but that problem was quickly remedied.
If you are ever certain that you got back a discard on the draw, I recommend that you stop immediately and call for a slot attendant. They can put the machine into diagnostic mode and display the five cards that were on the screen both before and after the draw. Most likely, you received a card of the same rank, but of the other suit of the same color.
If it turns out that you actually did receive a discard back, it is a malfunction. The machine should be immediately shut down for repair, and the minimum the casino should do is to refund your bet for that play. If they don't shut down the machine, and you're in a state licensed casino in Nevada, Louisiana or New Jersey, you should call for gaming control. In other places, who knows?
