First class flight rest room

Do you actually pay for your first class ticket or is it, like most, an
upgrade.

  From the airline employee's perspective--whether you pay for or get an upgrade (paid for by all the business you give that airline--like a casino comp!) or like me--you're up there because you work for the airline. . . . . . you expect first class service. I also believe that the FAA now mandates that passenger's use the facilities in the class of service--this is to prevent a constant flow of people up to the front so the crew/flight attendants can better monitor who's coming forward that may have ill intents to interfere in/with the cockpit and flight. . . . So if a FA tells you to use the restroom in your class of service--she/he is only doing their job. IF they let you sneak into the 1st class one--consider it a gift.

There is no congregating near the forward rest room. All passengers
are instructed to "use rest rooms in their ticketed cabins". And
the statement by the flight crew is usually to the effect that FAA
regulations are in effect.

And, why all the fuss about "holding it in" anyway? I have often
seen instances, from my 1st class vantage point, whereby the forward
restroom is occupied and all the ones in the rear are empty. We all
get to do our share of "holding it in", whether we can afford first
class or not. LOL.
