Is their a database for win/loss statements from
casinos?
Or, do I need to e-mail or write to each casino?

Giving them a call might work too.
I usually stop by their Rewards Club, fill out the Request form for
the Win/Loss statement.

I'm not sure I understand your first question.
However, every casino does not offer this service
(providing a W/L statement), I would think you have to
contact each one individually.
First thing I would do is see if there is a reference
to W/L statements on their web site.

Carlos, do you know which ones do not provide statements? I am having
trouble with PPE.
Someone else answered this a week or so ago. They
require a signature so at the least they have to mail
you something or fax you something for you to sign.
The accuracy varies a lot, I got a few for 2002 just
as an experiment, now I need them for 2003 with my
five W2-g's. Generally, they are very accurate for
machines, some table game records are pretty good,
others not, even if you were rated in your play.

EVERY casino that I was interested in getting a win/loss statement
from had a link on THEIR website to print out a request for a
win/loss statement. They all have to be filled out and signed. Some
casinos such as Stations and the Fiestas also require it to be
notorized unless you sign them in front of the slot booth person.
Generally, you need to contact each casino separately. Different casinos handle the request differently. Harrah's has a form on their site that, when submitted, will return a statement for all action at Harrah's properties. MGM Mirage and Caesars Entertainment properties need to be contacted individually. Caesars has a form that they will E mail in PDF. Paris-Bally's has an option where statements will automatically be mailed. Some require a signed letter or form (Caesars, Palms) and other casinos (Stardust, Bellagio, GN, Aladdin) will pop one in the mail with just a phone call.
