At least in November, anytime in your birthday month you could swipe at the kiosk at Gold Coast, Orleans and Suncoast for your gift. Don't know about Sam's Town. Got $5 free play at Gold Coast, $50 food at Orleans and $25 food at Suncoast. YMMV maybe depending on recent play and/or card level.
Also got 2500 points in November after earning one point at South Point.
All other birthday offers were by invitation only.
