Yes, Five-play MS machines exist, and yes, they can eat your bankroll
very efficiently.
You can play just one "play" (that is one 4-level MS game) rather than
all five, but the graphics suck and you lose a lot of the wonderful
auditory parts that help to make the single- play so much fun.

And not only is there such a thing, but at Peppermill (at least as of
~6 weeks ago), 5-play is the only full-pay form of Multistrike we
found. We looked very carefully but were unable to find any regular
Multistrike with full-pay games such as 9/6 JOB.
5-play Multistrike is very fun, but can be very expensive at $25/pull
fully-loaded, quarter denom. You can easily drop a few hundred bucks
in a couple minutes. You can also easily win hundreds of dollars on a
single pull of 25 cent JOB, without hitting a royal. Huge roller
coaster -- enjoy the ride.
