I may be doing something wrong but according to Winpoker both hands
call for the same ST4 since both are open ended straights with 8
cards to fill the hand available.

Winpoker says the following in details:

First Hand
2.1138 for 4,5,6
1.7021 for 3,4,5,6

Second Hand
2.5532 for j,10,9,8
2.4884 for T,9,8

If you look at other EV charts, the 3,4,5,6 also shows lower than 4,5,6.

In DEUCES WILD(DW), 3, 4, 5, 6 is not an inside straight. It should
be considered an outside straight because the 2 interferes with the
straight. In fact, my Jazbo card treats it as such. Of course, that
doesn't mean that I don't get this wrong a lot.

I think you may have your terms crossed. An outside straight refers
to a consecutive run of cards without a break (e.g. 5 6 7 8). An
inside has a break (e.g. 5 7 8 9).

In DW, (3 4 5 6) is treated as an inside straight because it has as
many opportunities for completion as a standard inside straight:

For example:

(5 6 7 8) can be completed by 3 cards, 4 9 or 2.
(5 7 8 9) can be completed by only 2 cards, 6 or 2.
(3 4 5 6) can also only be completed by 2 cards, 2 or 7.
